I never thought about it like that. So do you think that Helen telling Marc that she wasn't in love with him was to make him go to the Balkans because she knew that if their relationship didn't work out, he would regret it forever?

Could be. After all, Marc as good as said that their relationship was the only thing stopping him from going, so poor Helen would have been under a lot of pressure, constantly wondering if he still wanted her or if he would have preferred a life in Kosovo. On the other hand, Helen's marriage had been through a rough patch (due to her affair with that other bloke) shortly before she met Marc and her husband died while she was sleeping with him. Under the circumstances being in love with him might have seemed wrong to her at the time. I wouldn't be surprised if Helen had spent so long pretending that she wasn't in love with Marc, that when she was put on the spot, as it were, she had convinced herself it was true. I have to say though that Marc dropped considerably in my estimation, during that episode. He behaved appallingly toward Helen, without even trying to understand what it was like for her. He said some really horrible things to her, slammed the door in her face when she tried to explain things to him and generally acted like a spoiled brat whose favourite toy had been taken. Perhaps, if he'd been more mature about things, they could have come to a compromise (e.g. Marc goes to Kosovo for 6 months and, when its over, decides whether the job or Helen mean more to him). It would have been a big risk for them to take but at least they could have parted as friends, if nothing else. By the way, did Marc ever apologise to Helen for that incident?

No! Although he did send her a birthday card soon after he left so maybe they made up off screen, but it was her that kept apologising when he returned and he seemed to maintain that she drove him away. I never thought of him acting like a spoilt brat before but it's so true. He said Helen never gave them a chance to fall in love but they hadn't been together properly for that long when they split, so maybe he didn't give her enough time to overcome all of what happened in the past and allow her true feelings to come through before giving her an ultimatum and heading off to Kosovo. If being in love with someone is loving them enough to let them go, then Helen was in love with Marc. When she was supposed to be going to Australia, he kept asking her not to go and to stay for him. Starting to change my opinion of Marc and Helen's relationship history!

I saw the episode with Helen's birthday, the year Marc went. I didn't realise he'd sent a card.

I'm pretty sure he did. It wasn't her actual birthday episode because she was wearing a nice dress for the occasion that day but no-one at the surgery remembered and she just had celebration with her kids. The way they found out they'd missed it was that Sarah accidentally opened the card from Marc the next day thinking it was just mail for the surgery. It said Happy Birthday Helen in whatever language they speak in Kosovo with it translated underneath and had a letter inside saying that the orphanage had been granted more funding and that he was asking for everyone. Then the others gave Helen a cake and bought her vouchers for a spa.
