Julia and Mac return from their holiday in Canada while on holiday Mac fell down and bruised his backside and Julia insists Faith has a look at. George has taken maternity leave early, Julia panics to find a locum. Jimmi tells Mac that Amanda has cancer and hands in resignation.
Meanwhile, Ella Staines is in her 50's and has recently lost her husband. Celia Cross is Ella's best friend and has noticed that Ella no longer wants to go out with her, and is reluctant to take part in any of their regular activities. The truth of it is, Ella can’t afford to go out, her husband has left her financially broke and she is too ashamed and embarrassed to tell anyone. Faith notices from Ella's medical notes she has missed her appointments at St Phil's Hospital for her hip resurface operation and tries to reassure Ella it would be the best thing for her -Ella is not interested and only wants a repeat prescription for her arthritis.
Faith thinks she may be afraid to have the operation and suggests she sees Greg and discuss the matter further. Ella’s loves her dog Bingo dearly. He also has arthritis and because she can’t afford the vets bills she is sharing her medication with him and delaying her own operation – once she has the operation she wouldn’t be able to get the anti-inflammatories for Bingo – Greg gives her a repeat prescription and makes her promise him that she needs to re-book her hospital appointment. Ella goes home. While curling her hair Ella hears a message on then answer machine from Celia.
She doesn’t want her to come and races down the stairs to tell her not to come, unfortunately she falls down the stairs and hurts herself. Once Celia arrives Ella insists she contact Faith and not call an ambulance. Faith manages to convince Ella that the hip operation will improve the quality of her life. she will no longer be in pain and will be able to take better care of Bingo who will be able to get free medically check ups which are available for people in her situation. Ella agrees and realises how silly she has been.
'Secrets'

Ronnie and George are busy sorting out the baby’s nursery but George can’t seem to decide where she wants things or what colour the room needs to be painted. Sarah over hears Mac and Helen talking about Jimmi and discovers he has resigned.
She comforts him and tries to get him to change his mind. Helen has been offered a new job as the Chair for the Juno Health Initiative and George encourages her to take it. – While Julia has taken on a new locum Marc Eliot.
Meanwhile, Tim and Caroline are in their 30’s they are expecting their first child. Tim is the curator at St Hugh’s Drop In Centre - his wife Caroline is heavily pregnant and regularly helps out . Caroline’s blood pressure is high and Mac has told her to take it easy but Caroline hasn’t really taken his concerns seriously. Sean Daly is an ex boyfriend and comes into the drop in centre which puts Caroline on edge – Sean is part of a past life Caroline wants to forget – she had a child with him which she gave up for adoption at the age of 16 and to make ends meet worked as a prostitute.
Sean threatens to expose her past. Caroline confides in Mac who suggests she tells her husband the truth. Sean follows her home and forces himself into the house out of desperation she calls Mac as she doesn’t want to involve the police. Mac manages to scare Sean off. Caroline goes into labour. Tim already knows about her past – Caroline’s mother had told him everything just before she died.
'The Family Unit'

Mac gives Dr Marc Eliot the new locum a tour around the Mill Health Centre. All the staff are pleased to have him back. Sarah telephones George and lets her know who the new locum is - only for her to rush down to the surgery to speak to Helen. Helen is not giving much away but George is adamant she is going to get the gossip. Bella is also putting pressure on Helen to make a decision on the job offer that has been made to her. Greg and Faith go out to lunch with Marc they are trying to find out the real reason why Marc has returned.
Meanwhile, George and Ronnie attend an anti- natal class much to the resentment of Ronnie. – George has given strict instructions to him not to reveal her identity as a Doctor. She wants to be known as another pregnant woman. The classes are run, in a big private house by a woman called Maddy. Maddy is a great believer in natural childbirth and implements this within her sessions. Maddy doesn’t have contact with her own son because he is Gay.
Ronnie is paired up with Cathy - she is 20's years old and is carrying the baby for a Gay couple who will pay her. Cathy tells Ronnie she is a surrogate mother for this Gay couple and will get paid for it. Maddy over hears this and tries to convince Cathy to keep the baby. All of this triggers Cathy’s contractions, George’s cover is blown and she manages to calm Cathy down. George and Ronnie help Cathy out of the house only to discover Simon, Maddy's son waiting outside. He is the father of the child – Maddy is disgusted with this but George tries to make Maddy understand that her son’s sexuality is not a crime and her loving him shouldn’t stop.
'Lost For Words'

Shirley Carver is in her 50’s she is the manager of Wetherby Residential Home. Shirley loves her job and the people she works with but has decided to leave, to take care of her elderly mother in Bournemouth. Her mother refuses to move into the home Shirley runs, and wants to spend what time she has, in her own house. Shirley calls Julia to help her out with a couple of things on the computer, being old friends Julia agrees. Julia hitches a lift with Marc who is also making his routine check ups at the home.
Ron is the handyman at Wetherby Residential Home. He really likes Shirley but is unable to tell her his true feelings – and now feels it’s too late. He suffers from a bad stammer which gets worse when he is nervous, this is another reason he has not said anything. Brigie Callaghan is long time patient at the home and has been watching Shirley put things in her car all day from her window. She knowingly pops a Brazilian nut into her mouth and then collapses – she has a serious nut allergy. Marc, Julia and Shirley are shocked that Brigie has taken such drastic action to make Shirley stay.
While preparing for the leaving party Ron tells Julia about his feelings for Shirley Julia convinces him to tell her. Shirley returns from the hospital after seeing Brigie equally confused she doesn’t know what to do. Julia is hoping Ron will say something and advises Shirley to have a good think about her life and what it is, she really wants from it. Shirley feels obligated to take care of her mother and with no other family in Letherbridge she has no reason to stay. Just as she gets ready to leave Ron declares his love for her and without a moments hesitation she decides to stay.

Marc and Helen finally get time alone to talk and catch up. Just as they are getting close, they are interrupted by a visitor - Marc's fiance, Cara...
Meanwhile, Mac tries to persuade a man in his late 30s to have treatment for testicular cancer, but he doesn't want his new young wife to find out about the illness, fearing she'll leave him if the treatment affects his performance in the bedroom. When he is taken ill in her company, he is forced to admit the truth. But his wife insists sex isn't important, and reassures him she'll support him through the treatment.
